Our unique Smart Care Assessment helps to identify the physical care needs of each individual, allowing us to get the right personalized plan of care in place. Once a person’s needs and risks are identified, our certified care staff develop and follow plans of care that concentrate on:

Physical (Personal) Care – delivered by thoroughly screened and highly trained staff which includes but not limited to:

  • Hygiene – bathing, grooming, incontinence care, oral care
  • Light Housekeeping, laundry, linen changes
  • Meal Preparation, intake, and output monitoring, feeding
  • Medication Reminders
  • Vital Signs, monitoring, and reporting changes in medical/cognitive conditions
  • Assist with all Activities of Daily Living (ADL’s)
  • Companionship, outings, visits/trips, doctor visits

Socialization – lack of social support is related to negative impacts on health and overall well-being, especially for older people. Having a variety of positive social supports can contribute to psychological and physical wellness of elderly individuals. Support from others can be important in reducing stress, increasing physical health, and defeating psychological problems such as depression and anxiety.

Companion Care

Companion Care makes each visit meaningful and is a valuable benefit to older adults who face challenges keeping up with daily household chores, are at risk for isolation by providing socialization, trips outside the home, emotional support, activities engagement, nutrition, healthy meals and meal planning, laundry and so much more. Our Companion Care program incorporates those daily activities that make a person’s life fulfilled, a sense of purpose while enabling them to remain independent in their own home.

No matter the physical or cognitive challenge we incorporate social activities into our plans of care. Each person is different, their social activities like their plans of care are tailored to their specific needs and interests. Our staff have access to an Interactive Activity Resource Center.
